Section 1 information must be completed by the first day of employment and Section 2 must be completed within three days of the hire date.
Criminal Background Check - must have satisfactory results from the background check performed within first five days of employment. Initiated with Human Resources by your department contact.

Submitting documentary proof of vaccination: All employees of SPU are required to be fully vaccinated against COVID-19 and to submit proof of vaccination unless the employee has been approved for a medical or religious exemption. See our vaccination requirements: FAQ's page for more information. If you have established your SPU username and password, you can submit proof of vaccine or an exemption request through our Patient Portal. If you do not yet have your SPU username and password then please follow these instructions: Submit hard copies of proof of vaccine to the black records box on the wall outside of Health Services clinic door. It is secure. Records can be submitted there, faxed to 206-281-2674, or mailed to Health Services (3307 3rd Ave W., Ste 110, Seattle, WA 98119).
