HSA Tax Reporting Requirements

General Information on HSA Tax Reporting


The IRS says that you must file Form 8889 if you (or someone on your behalf, including your employer) contributes to your HSA. Form 8889 must also be filed if you take funds out of an HSA, even if you don’t make a contribution to the HSA during that tax year.

While we cannot provide tax advice, we can provide some information concerning the W-2 you may receive for payroll deductions and how it applies to Form 8889. If you need tax advice, please consult a qualified tax advisor or the IRS.

  • If you are receiving contributions from SPU via payroll deductions, your payroll deductions for the HSA account will be shown on your W-2 in box 12, marked code “W.” Because your payroll deductions were taken pre-tax, they are considered “employer contributions” and are to be entered on line 9 of Form 8889. Since payroll deductions for the HSA were taken pre-tax, you will not be able to claim a deduction for those funds on line 13.
  • If you made after-tax contributions to an HSA (sent a check or transferred money from your bank account, for example), those contributions should be eligible for a tax deduction. See the IRS instructions for additional information on above-the-line deductions.
  • If you took funds out of your HSA account, you will receive a 1099-SA form reporting the distribution of funds from your HSA. These distributions from your HSA will be entered on line 14a of Form 8889.

If you have unreimbursed qualified medical expenses, they may be entered on line 15 of Form 8889. Please see the IRS instructions for an explanation of what may be included as an unreimbursed approved medical expense.

HSA Tax Documents


Your tax documents will be issued either via mail or posted in the customer website based on your delivery preference on record with HSA Bank. If you’ve not yet logged into the customer website via HSABank.com, your tax documents will be delivered via paper in January.

  • Form 1099-SA: will be issued in January. This reports all distributions/withdrawals from your HSA. If you did not have any distributions/withdrawals from your HSA, then a 1099-SA will not be issued.
  • Form 5498-SA: will be issued in May. This form reports all prior tax-year contributions to your HSA.


How do I access my HSA tax forms?


Depending on the preferences set up with the HSA Bank, you may be receiving your tax forms in the mail in January. They are also available online in the following areas:

  • Form 1099-SA: you may log into HSABank.com, select the “Review My Coverage” tab, click on “Health Savings Account” and “Visit your HSA bank to manage your account.”  This will redirect you to the HSA Bank site.  From there, navigate to “Statements & Notifications” and click on “1099-SA (*year*)."
  • Year-end contribution information: Log into HSABank.com, select the “Review My Coverage” tab, click on “Health Savings Account” and “Visit your HSA bank to manage your account.”  This will redirect you to the HSA Bank site.  From there, navigate to the Accounts tab, and clicking on “HSA Contributions by Tax Year”. By clicking on “View Example”, you will be shown how to calculate your year-to-date totals.
